At Tri-State Surgery Center your safety is our priority. In order for us to prepare for you and your procedure we need to know about your health history, medications, and previous surgeries. Our patients are offered the convenience and privacy of a secure Patient Portal.
The link to your Patient Portal will be sent via text and or email.
To prevent cancellations or delays log on and COMPLETE
your Pre-Surgical Questionnaire as soon as you receive the link.
Your responses will be reviewed by a nurse, who will contact you by phone if there are any additional questions.

Why should I use the portal?
Many patients prefer to complete their questionnaire on their own rather than answering questions over the phone. You can view your Pre-Surgical Instructions at any time by clicking on the link in the email or text message.

No, by clicking the link in the email or text message, you will be broug to the login screen in your web browser .

Do I need to create an account and password?
No, your secure link will allow you to log in using your name and date of birth

How do I get a link?
The link will be sent automatically if a cell phone number or email was provided by your surgeons office. If you have not yet received your link call 724-225-8800 ext. 113 to verify your information and have a link sent.
